Department of Transportation (D.O.T.) Physicals
ATTENTION - TRUCK, VAN, TAXI, and BUS DRIVERS
In order for you to renew
your commercial license, the Department of Transportation requires that you have a physical exam every two years. The
physical exam enables you and your healthcare provider to update your health profile.
To
save you time and to serve you better, we ask that you bring:
- Your
eye glasses
- A list of all over-the-counter medications you are taking including
herbs and vitamins

A list of your prescription medications and their dosage
To provide you with quality
care, we ask that you:
- Complete a form with demographic information
and a brief health history
- Provide a urine specimen to check protein and glucose
levels
- Provide a urine specimen for a drug screen if your employer requests
To give you our professional best, during your exam we will:
- Check your height, weight, and blood pressure
- Test your vision
and your hearing
- Examine your eyes, ears, nose, throat, heart, lungs, abdomen
- Assess your vascular system, musculoskeletal, and neurological systems
- Check for tuberculosis (TB) if required
You will receive a Medical Examiner's
Certificate when your exam is complete. Keep this certificate with you when you are driving.
